(06/19/09) Note to Forms-on-CD purchasers: If you ordered a cd in the past and are having problems due to the new Internet Explorer 8, download a new forms cd here: Forms-on-CD download site. Be sure to uninstall your previous Forms-on-CD. Contact the first tuesday staff at 800-794-0494 if you need assistance.  

Forms-on-CD Version 4.2 Minimum System Requirements:

1. 500 MHz or faster processor running Windows® 2000/XP/Vista.
2. Pentium II or higher processor with 256 MB RAM or more.
3. 550 MB of available hard-disk space.
4. CD-ROM or DVD drive.

Forms-on-CD 4.2 Installation from CD:

1. Insert Forms-on-CD into your CD-ROM drive.

2. Click the "ftFormsCD.exe" icon. Automatic installation will begin.

Note: If automatic installation does not begin, please see Alternative Installation Instructions.

3. Click next in the “first tuesday Forms-on-CD Setup” window and follow the onscreen instructions.

4. Double click the "first tuesday Forms-on-CD" icon on your desktop to start the program.

Forms-on-CD 4.2 Installation from Download:

1. Follow the link provided in your email confirmation.

2. Enter your DRE license # or T # to log in.

3. Click the "Download Now" button to begin download.

Note: The download process may be lengthy depending on your internet connection.

5. On completion of the download, a "ftFormsCD.exe" icon will appear where the software was saved. Click the icon to begin automatic installation.

6. Click next in the “first tuesday Forms-on-CD Setup” window and follow the onscreen instructions.

7. Double click the "first tuesday Forms-on-CD" icon on your desktop to start the program.

Getting Started - Forms-on-CD 4.2:

1. Double click the “Forms-on-CD” icon on your desktop.

2. When the icon is clicked, the Forms-on-CD Welcome Page will appear. From the Welcome Page you can access form indices, reading materials, instructions on how to use the Forms-on-CD, and other useful features.

3. Links which can be followed appear in blue. Click on the link to follow it.

Note: To follow a link, your filled-out form should be printed or saved; otherwise, your data will be lost.

 4. To return to the Welcome Page, click “Back To Welcome Page” on the top of the page.

Pre-Installation Troubleshooting:

Forms-on-CD won’t install.

1. Remove CD from the CD-ROM drive and restart the computer.

2. After restarting, reinsert the CD.

3. Click the "ftFormsCD.exe" icon. Automatic installation will begin. If it does not, see the Alternative Installation Instructions below.

Alternative Installation Instructions:

1. Insert CD into CD-ROM drive.

2. Right-click Start button. Select Explore.

3. Click the CD-ROM drive where the disk is located.

4. Double click the "ftFormsCD.exe" icon. Follow the onscreen instructions.

5. Once installation is complete, a “first tuesday Forms-on-CD” icon will appear on your desktop.

6. Double click the "first tuesday Forms-on-CD" icon on your desktop to start the program.
